Photography consent

The school asks these questions before using photos on the website or social media:

• If you are under 18, a parent or guardian completes and signs the waiver.
• First name, last name, date of birth, and phone.
• Are you a parent or guardian of a student?
• If yes, the children’s names and ages, so they can be included.
• Do we have permission to take photos of you or your children and use them for the website or social media?
• Signature.

This preview does not collect a signature. Ask at the school, or write cballard@kattaekwondo.com.
